Our Community Connections service provision came to an end on the 31st October 2022.
Shoreditch Trust supported people to both prevent and manage long-term physical and mental ill health. This included support for those who have multiple health conditions, often compounded by social isolation and complex wider support needs.
We worked collaboratively with clients, family, carers and a range of providers to support people to recover from life changing health events and to promote positive strategies for maintaining health and wellbeing.
The programme offered one-to-one coaching and groups to support positive changes for Hackney residents aged 18+. A longer-term programme with more extensive support was offered to people aged 50+ who experienced social isolation or feeling cut off from their communities.
Help to get out and about: The Shoreditch Trust Community Connections team know all about the exciting activities happening in Hackney, from bingo sessions to free lunches. We supported residents to get involved, make connections and take an active role in their health and wellbeing.
Chance for a chat: We also offered telephone befriending through a service called 'We Connect' - a regular telephone call from a local volunteer.
